SunCare Home Health sends skilled health care professionals such as nurses and therapists to your home. Under orders from your own doctor, we can provide a number of different services designed to improve your health, prevent future problems, and/or shorten your stay at a hospital or nursing home.

Who Qualifies for Home Health

Medicare requires that patients meet the definition of homebound. This simply means that, in your current condition, leaving home requires a considerable and taxing effort and that you therefore leave the home infrequently. It can also mean that your doctor has ordered you to not leave the home for a while due to your current medical condition. Homebound is not the same as bedbound. Most homebound patients can still move about their homes, and some homebound patients can even go for walks.

Home health services also require that a patient have a "skilled need." This means a patient must need more than our aide services that assist patients with activities of daily living such as cleaning, bathing, grooming, and light meal preparation. Skilled need means you need the services provided by nurses and therapists such as diet teaching, exercise design and supervision, medication teaching, collecting lab specimen, or skilled observation.

Finally, home health services require the authorization of a physician. SunCare Home Health does not require patients to see a new physician. Instead, we believe it is better for us to work with your physician whom you already know and trust. When patients are Medicare beneficiaries, physicians can and should bill for the work of coordinating care with SunCare Home Health.

How to Get Started

  • Anyone can make a referral for home health. Help is just a phone call away!
  • Referrals are also made by discharge planners and social workers.
  • A doctor’s order is required in order to provide home care that is covered by insurance.
  • For your convenience, we will coordinate all care directly with your doctor. Simply provide us with the name of your physician when telling us about your health care situation.


Payment

For qualified patients, Medicare pays 100% of allowable charges from SunCare Home Health. Most insurances cover home health services. Please contact us to find out if we currently participate in your insurance plan.

 

Assisted Living Facilities

SunCare Home Health is well versed with all clinical and management issues, as well as Medicare and Michigan guidelines, when it comes to providing services to assisted living facility (ALF) clients. When SunCare Home Health teams up with an assisted living facility, we help that ALF stand above other facilities in the area. SunCare Home Health helps select Michigan assisted living facilities:
