The Suncare Home Health Team Includes:

• Physical Therapy: Designs and monitors home exercise programs that can restore strength, flexibility, and coordination. Designs, implements and monitors pain relief programs. Develops strategies for training to improve gait and balance, thus preventing falls. Prescribes plans that aid in recovery from stroke.
• Occupational Therapy: Teaches activity strategies and home modifications to overcome disability. Provides direct rehabilitation for complex areas of the body such as the hands. Teaches proper use of assistive equipment.
• Speech Therapy: Designs and monitors home programs to improve speech, eating, and cognition.
• Skilled Nursing: Provides direct medical care such as wound care, injections, and collecting of lab specimens. Provides individualized health education on health matters such as new medication regimen, diet teaching, and teaching family to best care for a loved-one.
• Certified Nursing Assistants: Provides in-home assistance with activities of daily living while patients are ill or recovering. For instance, cleaning, making the bed while patient is still in it, bathing, grooming, patient's laundry, light meal preparation, etc.
